Therapeutic apheresis modalities, including therapeutic plasma exchange and extracorporeal photopheresis (ECP), are increasingly being utilized as part of the multidisciplinary care of solid organ transplant recipients. This session will compare and contrast different treatment regimens currently employed for preventing and treating solid organ transplant rejection. We will review the indications for initiating treatment with apheresis and the impact on patient outcomes. We will review the pathophysiology of organ rejection and more recent data on using ECP to induce organ tolerance. We will highlight ECP's role in managing chronic lung allograft rejection.

Learning Objectives:

  • Describe the pathophysiology of organ rejection, therapeutic options, and proposed mechanisms of action of extracorporeal photopheresis and therapeutic plasma exchange
  • Contrast different treatment protocols using therapeutic plasma exchange and extracorporeal photopheresis in the management solid organ transplant rejection
  • Compare outcomes of apheresis-based regimens in the treatment of organ rejection
  • Examine the data supporting the ability of extracorporeal photopheresis to induce organ tolerance
  • Recognize the emerging role of extracorporeal photopheresis in the treatment of chronic lung allograft rejection (CLAD)
